Every Camper that wishes to attend Camp Northfield must provide accurate and up-to-date medical information. The health and safety of each and every camper and staff member is our top priority. Every camper must show proof of a physical preformed within 24 months of the start of their camping week. Forms can be mailed to Camp Northfield or emailed to registration@campnorthfield.com.

For each prescription medicine that you will be sending with your camper please fill out and bring one (1) Medication Authorization form. This form is only needed for prescription medications.
